MyUFHealth is a patient portal that let you or someone you give permission to see parts of your medical record from a computer or mobile device. - Go to Mychart.shands.org to access the official MyUFHealth login page.
- In the input field, please enter your MyUFHealth Username and Password.
- Then, to access your MyUFHealth Portal, click on the “SIGN IN” option.
How do I sign up for the MyUFHealth Portal?
To successfully register your MyUFHealth account, please follow the simple procedures below:
- Go to Mychart.shands.org to access the official MyUFHealth login page.
- Now, as indicated in the screenshot above, click the “SIGN UP NOW” button.
- Now, input your MyUFHealth Activation Code, Zip Code, and Date of Birth, then click Next and follow the on-screen instructions to create your account.
What Should I Do If I Forget My UFHealth Password?
To successfully forget your MyUFHealth account password, please follow these simple steps:
- Go to Mychart.shands.org to access the official MyUFHealth login page.
- Please select “FORGOT PASSWORD?” from the drop-down menu. As shown in the screenshot above.
- Please enter your MyUF Health Username and Date of Birth and then click Next.
- Please follow the instructions on the following page to reset your password.
